Why most agentic AI projects fail before a single line of code is written

The numbers are difficult to ignore. According to S&P Global Market Intelligence's 2025 survey of more than 1,000 enterprises across North America and Europe, 42% of companies abandoned most of their AI initiatives before reaching production, more than double the 17% recorded the previous year [SPGlobal]. On average, the organisations surveyed scrapped 46% of their proof-of-concept projects before they reached production.

These are not technology failures. Gartner analyst Anushree Verma put it plainly in October 2025: most organisations cannot operationalise their agentic AI projects, not because the AI falls short, but because the strategy behind it does [Harvard Business Review].

And this is something we at Vstorm recognise immediately. Over 30+ production deployments across engineering, healthcare, telecommunications, and print-on-demand, we have seen the same pattern repeat. The projects that reach production share one thing: a structured planning phase that happened before engineering began. The projects that stall share five specific failures. How most organisations approach agentic AI strategy today #

The typical pattern looks like this. An executive sees a competitor announcement or a vendor demo. A target process is identified. An internal IT team or an external development partner is engaged. And engineering conversations begin within weeks.

What is usually missing at this stage is any structured assessment of whether the selected process is actually the right automation target. No-one has defined what the agent needs to do at each step. No-one has checked whether the data is accessible and reliable. No-one has named the person who will own the deployed system once it is running.

Process documentation, where it exists, describes how the workflow is supposed to work, not how it actually does. Agentic AI use case selection is a genuine requirement. This means operational analysis, technical feasibility assessment, and ROI modelling per use case. Most organisations undertaking their first or second AI transformation do not have that capability in-house. And the consequences of that gap are now showing up clearly in the data.

Five planning failures that precede project collapse #

Across 30+ deployments, we have identified the same five failures appearing repeatedly, in different industries, at different scales, with different technical partners. None of them are accidents. And none of them require a technology solution.

1. Undefined success criteria

The project launches without measurable outcomes attached to it. When review arrives, there is no basis for determining whether the system is working. The team cannot answer a simple question: compared to what?

S&P Global found that organisations with lower project failure rates are significantly more likely to evaluate compliance, risk, and data availability at the use case selection stage, before engineering begins [SPGlobal]. The organisations that skip this step build a lot. They just cannot tell whether what they built is working.

What this looks like in practice #

Synera operates an AI agent platform for engineering teams used by organisations including NASA, Airbus, and BMW. Their platform lets engineers build complex automation workflows using a graphical, node-based environment. The problem was adoption: creating those workflows required deep familiarity with the platform, which created barriers for new users and training overhead that cut into time better spent on design.

The question to ask before any AI engineer writes a line of code #

The AI pilot to production gap is not a technology problem. It is a planning problem. And it is almost always visible before the build begins, if someone knows where to look for it.

Before committing an engineering budget, three questions should have clear answers.

• Who is the operational owner of the deployed system?
• What does success look like in measurable terms at six months post-deployment?
• What are the primary data sources the agent will rely on, and have they been assessed for quality and accessibility?

If any of those is unanswered, the build will surface them as problems. The project will slow down, rework will accumulate, and the resulting failure will be attributed to the inadequacy of the technology by default.

But understanding why agentic AI projects fail usually leads to a simpler conclusion: the planning layer was absent, or it was conducted by people who were not responsible for building what it described.
